Social Workers – Adults in Transition (1 Year Fixed Term Contract)
An exciting opportunity has presented itself within Adult Social Care for 2 new social workers to work in partnership with service users with disabilities, their families and carers and local service provider agencies following their transition to adulthood. One social worker will be based in the North Community Locality Team in Finsbury Park and the other based in the South Community Locality Team based in Kings Cross.
The successful applicants will be working with service users with a range of complex needs, including autism, who do not meet the criteria for global learning disabilities and will be based within the North and South Community Locality Teams, working closely with the Integrated Learning Disability Partnership.
The post holders will be responsible for; undertaking Care Act 2014 assessments and accessible outcome focused support plans. They will also contribute to Education, Health and Care Plans (EHCP’s) and review service user’s needs to enable them to achieve their potential. Islington is committed to enabling people with disabilities to live the lives they want to live with the support they need in line with the principles The Care Act and Mental Capacity Act. Knowledge of working with this service user group would be an advantage and experience of working with adults in a personalised way is essential.
